More about the job:This position is responsible for comprehensive clinical service provisions for clients, as well as promoting family stability within a framework of strength based and culturally sensitive services in our Cincinnati division. This will be accomplished through the following: Complete referral reviews and intake assignments. Communicate service availability to all assigned clients promptly. Provide instruction, intervention and assistance as deemed necessary through the exercise of independent judgment and knowledge or as part of the treatment team to preserve the family unit. Coordinate with county agency as appropriate, family sessions to promote successful clinical interventions. Collaborate service provisions with school officials, courts and other appropriate parties involved with the client, according to policy, regulations and best practice standards. Attend hearings as assessed, assist client and families with community linkages. Provide after care services as required by contract. Provide Medicaid billable services to the youth with mental health diagnosis; promote daily living skills that impact delivery of mental health services which focus on self-sufficiency through specific outcomes, such as obtaining housing, job education or vocational training, counseling and community networking; providing outpatient mental health case management services. Complete assessments as required by state rules, agency policies, and/or contract requirements. Assessments may include psychosocial assessments, Diagnostic Assessment Forms, and Family Functioning assessments, Mental Health Scales. Complete required Outcome Measure reports. Prepare Individualized Treatment Plan (IAP, ISP etc.), DAF’s, and Mental Health Individualized Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state. Establish mutually respectful rapport and implements interventions necessary to meet needs of client/collaterals; appropriately sets limits and maintains helping role as a Mental Health practitioner. Assist the division for additional clinical services as designated by the Treatment Director. Timely document outcome of home visits, support sessions, interaction with parties involved in the youth’s care and incidents as required by policy, regulations and contract requirements. Prepare and submit timely all required company, RFP, program, referral agency and regulatory reports, FAF, progress notes, and billing tickets. Compile profile using pre and post assessment tools for outcome measures. Maintain contact with referral agency caseworker using discharge reports, Diagnostic Assessment forms, telephone calls, and incident reports in addition to regularly scheduled and required monthly written reports. Review all youth and foster home files pertaining to caseload as part of the quality improvement process. Inform clients and families of company policies, procedures, rights and responsibilities and incident reports. May be on call 24 hours to address crisis with clients and will carry the pager as assigned by Treatment Director.
