liaison between families children and hospital caregivers to ensure that discharge/outpatient care plans are appropriate, practicable and based on the family and individual’s strengths and needs
Participate in Family Team Conferences organized by foster care agencies, as well as discharge planning
Provide a combined child welfare/mental health care perspective during conferences
Establish linkages for specific children with appropriate community based services
Assist planning teams and other appropriate agency staff in developing appropriate care plans and linkages for foster care youth
close interaction with Conference Facilitators Assist children families and inpatient service providers with understanding implications of discharge/outpatient care and placement plans

Child and Family Health

The Office of Child & Family Health (OCFH), under the Division of Family Support Services, is charged with ensuring that all foster children served by Children’s Services have timely access to quality, comprehensive physical and mental health services, and that these services are provided in a continuous and coordinated manner. OCFH supports Children’s Services and its operations through quality improvement, research, education/training, advocacy, direct services and individual case consultation in the areas of health, mental health, substance abuse, and domestic violence. OCFH works closely with the Office of Policy and Planning in developing programs, practices and procedures related to health, domestic violence, substance abuse and mental health. Specific responsibilities of OCFH include: 1) informing child welfare and child care policies and practices with knowledge of clinical issues and perspectives in physical and mental health disciplines; 2) improving child welfare outcomes through better integration of medical, mental health, substance abuse services, domestic violence and child welfare services; and 3) working with the medical, mental health, developmental, substance abuse and other relevant service systems to strengthen partnerships and advance Children’s Services’ agenda.
The Mental Health Inpatient Coordination Unit (MICU) is responsible for ensuring linkages to appropriate community-based services for children, stabilizing placements and reducing recidivism from a hospitalization standpoint. MICU is designed to provide consultation, coordination and policy clarification to staff at Children’s Services, foster care agencies and psychiatric inpatient units. Reporting to the Assistant Director for MICU, the Inpatient Coordinators will provide specialized case coordination services for foster children and teens admitted to psychiatric hospitals. Coordinators will remain involved in cases from the start of the hospitalization until the time that the child is re-stabilized in an aftercare setting.
