Candidate must have good knowledge base for a high volume HMO population and extremely active rehabilitation units.
The Discharge Planner/Social Worker functions under the direct supervision of the Director of Social Services. Responsibilities include but not limited to, the coordination and implementation of post rehab care by evaluating and assessing the residentsí needs, financial status/insurance coverage and community resources available.
The Discharge Planner/Social Worker coordinates with physicians, Nursing, Case Managers/Care Coordinators and the residents along with their families/significant others who will be involved in their post-rehab care.
The Discharge Planner/Social Worker must maintain a high level of confidentiality, possess excellent written and verbal communication skills, and have sound judgment. Must also have the ability to develop and maintain effective working relationships and maintain a professional demeanor under stressful conditions.
